Community matrons

If you have a long-term condition, such as diabetes, multiple sclerosis or Parkinson's disease, community matrons will help to improve your health and keep you as independent as possible.

They will prevent you from being admitted into hospital unnecessarily by making it easier for you to manage your health condition. Community matrons will:

  • develop your own care plan with you, your carer, relatives and other health professionals, by assessing your medical, nursing and care needs;
  • refer you to other specialists if required, eg for tests;
  • review your medicine with the support of your GP, and inform other professionals of any changes in your condition;
  • teach you and your carers and relatives how to recognise changes in your condition that could affect your overall health, and when to call for help;
  • keep in contact with you if you are admitted to hospital and make sure the health professionals treating you have all the information they need for you to receive consistent care;
  • assist hospital staff in planning your return home; and
  • keep in touch with you by home visits or over the telephone.
